APPLICATION TO BE COMPLETED AND SIGNED BY THE “WELL OWNER”. “WELL OWNER” means the person who owns a possessory interest in: (1) the land upon which a well or well system is located or to be located; (2) the well or well system; or (3) the groundwater withdrawn from a well or well system. (A potential purchaser of the land, well/ system or groundwater is not the Well Owner)
